Our program is designed to meet specific dietary needs through medically tailored meals (MTM) and personalized medical nutrition therapy, aiming to enhance health outcomes and decrease healthcare costs within just three months. Participation requires a referral from a healthcare provider, ensuring that our services are targeted to those who truly need them. However, please note that we cannot accept individuals living in facilities offering more than seven meals a week, those with significant cognitive or physical challenges that may hinder participation, individuals with a life expectancy of less than one year, or those with severe food allergies.
