The ACEs Network of Care Clinical-Community Linkages Program, based in Clovis, CA, is dedicated to supporting Central Valley providers in connecting patients who have experienced Adverse Childhood Experiences (ACEs) to vital, trauma-informed services. Our compassionate team of Community Health Workers (CHWs) is committed to educating patients about their ACEs scores while facilitating access to a wide range of community resources, including healthcare, behavioral health services, food access, and social services. With a focus on enhancing protective factors and fostering safe, stable environments, we strive to address patient stressors and promote nurturing relationships.
