Aging and Disability Resource Connection

The Aging and Disability Resource Connection (ADRC) in Anaheim, CA, is dedicated to empowering seniors and adults with disabilities by connecting them to essential resources that enhance their independence. We provide valuable information about a comprehensive array of services, facilitating informed choices for care planning. Our compassionate team offers short-term service coordination to swiftly address urgent needs, helping to prevent unnecessary hospital stays or nursing home placements. We also assist with critical services including financial aid, in-home care, home modifications, assistive devices, and home-delivered meals. Additionally, we provide insurance counseling and transportation support, as well as guidance for transitioning from nursing homes back to the community.
