Survey for those with Disabilities or Chronic Conditions

Hello. I am a high school senior currently taking an Engineering class. Right now I am working on a project in which we work in teams to design and develop an original solution to a problem. I am currently working on finding a solution to assist those with disabilities or chronic conditions who may struggle to open doors. In order to develop an effective solution, we would like to hear input from those who we are making the product for. I was hoping you would be willing to complete the survey provided below (which will take less than 5 minutes) to provide us with information that will aid in the creation of an effective solution.
