...See more Good morning/afternoon/evening everyone!!! It's been a nice, quiet morning over here. I did laundry last night, and I've just got that taken off the drying rack and put away. I got my washing machine put away (I have a small apartment so I have a portable washer that hooks up to my faucet and drains into the sink), I did the dishes, and I got my kitchen cleaned up a bit - and it's not even 10am! After this, I have some homework to do, but it's just the first week work, so it's mostly introductions and syllabus quizzes, so not bad at all. BUT All that I've done this morning, and all I'll do the rest of the day, no matter how small a task it may seem, goes against everything I was made to believe about myself while growing up. I was always told that I was useless, worthless, stupid, how I would never do anything with my life, etc. I've built all the furniture in my apartment. I sponsor four children. I give as much as I am able whenever I am able. I have done so well in school I got an "academic honors incentive" added to one of my scholarships, which means extra money each semester. At work I constantly hear people say things like, "What would I do without you?" I am not useless - I am capable of a lot, and I prove that daily. I am not worthless - my life has value, and I do for others all the time. I am not stupid - I am actually quite smart. I have done quite a lot with my life, and it's really only getting started. Those are just four examples of negative beliefs that can be ingrained in us from a young age. So today, I would like to challenge each of you to share a negative belief that still haunts you, and to flip it around to be a positive, like I just showed you with some of my own negative beliefs. Whether it's something like "I'm not ugly - people always tell me how pretty I am.", or something like "I do have control in my life - look at everything I've done for myself!", please at least try to share just one negative belief you have. πŸ’œ Share a little Share a lot Share whatever you want in here Because this is YOUR. ...See more Hey guys!! Okay, I gotta make this short and sweet cuz my power went out so Im posting from my phone, which means I dont exactly have much room to work with. Today, for all you Americans out there, is the Fourth of July!! Now, what that has meant for me is that it sounds like bombs are going off basically 24/7 for the last couple weeks and, if the same thing happens this year as every year, I wont be able to see from all the smoke in the air later. Super great times. Whew. It is really getting hot in here. Okay. Anyway, I love fireworks, but only when I intend to hear them. All these unexpected and random booms have really been messing with my anxiety. More super great times lol! ...See more Good morning/afternoon/evening everyone!! It's midday for me, and I don't know about any of you, but I am TIRED!! I mean just absolutely drained. Because it's like 800 billion degrees outside, so I'm sitting here with no lights turned on, and a ceiling fan going above me, and three fans pointed at me, but I'm still just super exhausted from this heat! Now, normally, when I feel super tired like this, it's not heat related. It's because of all the reasons I found this forum in the first place. So that's what I want to talk about today. Well, sort of. I'm gonna let Savannah Lloyd from The Mighty do the talking for me. Here's her post, What "I'm Tired" Means for People With Mental Illness. [https://themighty.com/2016/12/what-im-tired-means-for-people-with-mental-illness/] Many people use the expression Im tired when theyve had a lack of sleep or when they feel like they need a nap. When youve got mental health problems, sometimes Im tired [https://themighty.com/2016/07/answering-how-are-you-with-im-tired/] can also simply mean youre lacking sleep, but often it means so much more. When I say Im tired, Im usually not just physically tired. Im emotionally tired. Im holistically tired. Im tired even when Ive spent the entire night sleeping in bed. Im tired even when I dont move all day. Its not just tired eyes and achy muscles. Its not just a yawn and just one more hour in bed. Its getting up and getting dressed in a blur. Brushing your teeth and brushing your hair, and then leaving the house. All whilst tired. Emotionally tired. Numb. Drained. Completely out of it. Lost. But you move on with the day anyway, because there seems to be little acceptance of what mental illness [https://themighty.com/mental-illness/] can do to your body. Not many people ask me if Im OK, but when they do my answer is always the same. Im fine, just tired β€” and people seem to accept that reply. Tiredness is an accepted feeling β€” everyone gets it. A long day at work or sitting through a boring lecture. Thats tiredness for many can relate to. But that tiredness isnt lying in bed all day and still feeling like you could sleep for a thousand years. For me, though, thats what tiredness is. Tiredness accompanies my depression [https://themighty.com/depression/] and my anxiety [https://themighty.com/anxiety/]. It means lying in bed completely exhausted from life without even falling asleep. It means being spaced out and lost in thought most of the day, because its tiring trying to keep up with people. It means achy eyes and yawns even after 12 hours of sleep. It means not just feeling physically tired, but feeling oh-so much more. When someone tells you theyre tired, sometimes you need to look beyond their answer. Are they tired? Are they physically tired and need some sleep? Or do they in fact need you. Do they need somebody to look them in the eyes and tell them theyre not fine but that youre there for them? Do they need someone to realize theyre not OK and to offer them a hug? Because I know when I say Im tired, thats what I need. I dont need sleep or a nap. I need people. I need love. I need understanding. What does it mean for you? I know for me, I'm usually just so drained, and I feel like I need to nap for like 84 years. But I also usually need help. Because I'm just so tired of having to do everything on my own. I'm so tired of having no one to do anything with. I'm so tired of having no one to come and help me get my place in order. Not to sound super selfish or anything, but we just took care of everything for the widow of one of our patients, and I couldn't help but think that I wish I had people who would come help me. She called us on Friday, and within a week she had food, furniture, bedding, her place was clean and organized, I spent about three hours over there helping her get all of her banking and everything set up on her phone, and I got her set up with a car service because she can't drive. She kept telling me how we were all God's answers to her prayers. She was just so overwhelmed with everything in the couple months after her husband had passed, and she just needed help getting herself back up. I'm overwhelmed. I'm struggling. I need help getting back up. But it's just me. I have to force myself to keep going every day. To get up, go to work, exist as a normal human. But I'm just so tired. That's what tired means to me. Tired means my stress cup is overflowing, and I just need some help bringing it back down. Tired realistically means my cup has been that way for a while, and I'm just running out of fight. ...See more Good morning/afternoon/evening everyone!! My apologies for the late checkin today. I was trying to think of what to do for today, and I think I want to use an article that I shared in another post yesterday. One of you was so brave in creating a post that talks about something I think is really important. So, I'm going to share this article [https://information.pods-online.org.uk/did-or-ddnos-does-it-matter/] about something called Dissociate Disorder Not Otherwise Specified (DDNOS) or Other Specified Dissociative Disorder (OSDD). Soooo...with no further ado, here we go: WHAT IS THE DIFFERENCE BETWEEN DISSOCIATIVE IDENTITY DISORDER AND DISSOCIATIVE DISORDER NOT OTHERWISE SPECIFIED (DDNOS)? I talk to a lot of people on the PODS helpline, via email, and at training days and one of the questions that comes up time and again is about DDNOS – dissociative disorder not otherwise specified. Ill explain technically what DDNOS is in a moment, but a quick (although inadequate) definition might be dissociative identity disorder without distinct parts of the personality. Many people with DID struggle with what their diagnosis means to them – they may resent it or disbelieve it, but there is at least some understanding, and an increasing amount of literature, on the nature of dissociative identity disorder. The same cannot be said for DDNOS. People with DDNOS often feel that their experience is not represented in books, articles and websites, that they are β€˜less than people with DID – that not only are they β€˜messed up, as one person put it to me, but, Weve even messed up being messed up, by not having a proper condition. DDNOS – THE MOST COMMON DISSOCIATIVE DISORDER? This is a painful position to be in, and yet a variety of studies have regularly found that DDNOS is either the most common or among the most common dissociative diagnoses: it is diagnosed, according to ONeil et al (2008), in 40% of cases. Suzette Boon reports that DDNOS actually involves the majority of people who seek treatment for a dissociative disorder (Boon et al, p.10). And Spiegel et al (2011, p.838) state that A review and analysis of DDNOS concluded that the majority of DDNOS cases are actually undiagnosed (or misdiagnosed) DID cases. So something is clearly going wrong. According to the American Psychological Association, the predominant feature of DDNOS is: a dissociative symptom (ie a disruption in the usually integrative functions of consciousness, memory, identity, or perception of the environment) that does not meet the criteria for any specific dissociative disorder. (APA, 2000, p.503) In other words, someone with DDNOS has dissociative symptoms but they do not meet sufficient criteria to be diagnosed with either depersonalisation disorder, dissociative amnesia, dissociative fugue or dissociative identity disorder. SIX SUBSETS OF DDNOS There are six subsets of DDNOS listed in the DSM-IV: People who appear very similar to dissociative identity disorder but dont quite meet the full criteria. These are subdivided into two types: a) people where two or more distinct personality states are not fully evident; and b) people who do not have the usual amnesia for important personal information. Adults with derealisation but not depersonalisation. People suffering dissociative symptoms who have been subject to periods of prolonged and intense coercive persuasion (eg, brainwashing, thought reform, or indoctrination while captive). Dissociative trance disorder. Loss of consciousness, stupor or coma that has not been caused by another medical condition. Ganser syndrome. DDNOS – β€˜NOT QUITE DISSOCIATIVE IDENTITY DISORDER? In practice, there seem to be relatively few cases of subtypes 2-6, whereas subtypes 1a and 1b are extremely common. These are often referred to as not yet or not quite DID – people who havent yet met the criteria for dissociative identity disorder but may well do so in the future, or people who have slightly atypical forms of DID, for example by not having amnesia. This of course begs the question of whether DDNOS-1 and DID are in fact the same thing, and just different points on a spectrum, and whether the diagnostic criteria for DID are too tightly applied. Certainly DDNOS is supposed to be a β€˜residual category to mop up the β€˜few cases of dissociative disorders that do not meet the mainstream criteria. But if up to three times as many people receive a diagnosis of DDNOS compared to dissociative identity disorder, it would suggest that the definition of DID is too narrow. This is certainly the view of a number of experts in the field. For example Colin Ross (2007, p.142) says: The dividing line between DID and most cases of dissociative disorder not otherwise specified is arbitrary. Most cases of DDNOS are partial forms of DID which lack either clear switching of executive control, full amnesia barriers between identity states, or clear differentiation and structure of identity states. They are partial forms of DID with the same patterns of childhood trauma and co-morbidity. Similarly James Chu (2011, p.53) writes: Dissociative disorder not otherwise specified (DDNOS) is a catch-all category for dissociative disorders that do not fall into other groups. However, included in the DDNOS category is a commonly seen group of patients who do not have the extreme identity separation of dissociative identity disorder, but who have a range of dissociative experiences and significant identity confusion and alteration. Patients with this kind of almost DID do not see themselves as having multiple identities, but frequently feel so differently at the time that they see themselves as a series of different β€˜mes (eg, I know it was me, but I felt as though I was observing myself. I couldnt believe what I was saying and how I was behaving.) Also included in the DDNOS category are atypical DID cases in which there are classic DID symptoms but no amnesia between identities, because the DSM-IV diagnosis of DID includes the requirement for the presence of amnesia. DDNOS AND DISSOCIATIVE IDENTITY DISORDER – A SPECTRUM? So on the one hand we have a vast swathe of people who are, or would be, diagnosed with DDNOS as opposed to dissociative identity disorder but who show almost all of the symptoms of DID. Many people therefore see DID and DDNOS as appearing on a spectrum, and prefer to conflate the two conditions so that DID/DDNOS represents a range of dissociative experiences with more or less amnesia and greater or less elaboration and distinctive identity states or parts of the personality. It is also what happens in practice: very few people would realistically distinguish between DID and DDNOS – in PODS we certainly dont make a distinction and try to ensure that we are addressing the whole range of symptoms and difficulties in living with a dissociative disorder, rather than focusing either exclusively or predominantly on β€˜parts. We see the presence of these dissociative parts of the personality as really important, and of course it is the stand-out feature of DID, but we also recognise that parts develop in response to trauma and disorganised attachment, as do a whole range of other symptoms. So our focus is on living with that entire range of symptoms, and being able to deal with the underlying cause. THE IMPORTANCE OF VALIDATING DDNOS At the same time, however, it is important to note that there are differences between someone whose constellation of symptoms would identify them as being at the DID end of the spectrum, and someone whose similar but slightly different set of responses might place them at the DDNOS end of the spectrum. When there is often a strong emphasis on the dissociative parts of the personality, people with DDNOS can feel unheard and unseen, and so in PODS we feel that it is very important to validate the reality of the experience of people with the DDNOS label. We have touched on two major differences already – less elaboration or switching to distinct parts, and less amnesia. But people may be diagnosed as DDNOS as opposed to dissociative identity disorder simply because their β€˜parts didnt show up on cue at a diagnostic interview. Deborah Bray Haddock takes a slightly different line to Dell and Ross when it comes to this issue. She says: My advice to clinicians is that until they have met an alter, it is not DID. They may suspect that someone has DID and their suspicions may prove to be correct, but each of the four criteria must be met to diagnose someone with DID. Until that time, a diagnosis such as dissociative disorder not otherwise specified (DDNOS) might be more appropriate. (2001, p.9) This is a complex area of debate, because as Spiegel et al, in their paper Dissociative Disorders in DSM-5 (2011, p.839), point out: According to Kluft and Dell, only 15% of dissociative identity disorder cases regularly manifest easily observable alternate identities during diagnostic interviews. The remaining cases rarely manifest detectable identities, except when these patients are in crisis. Kluft used the term β€˜window of diagnosability to capture the latent nature of clear-cut switching phenomena in dissociative identity disorder patients. So for some people, their alters or parts are only obvious to other people during times of crisis. Does that mean that they are DID when they are in crisis but DDNOS the rest of the time? Surely not. Many commentators such as Dell and Kluft argue convincingly in a number of places that switching is hard to detect, and one of the least frequent β€˜signs of DID, and should not therefore be a core diagnostic criterion. It can therefore be very frustrating for some people with the DDNOS label not to have the full diagnosis of dissociative identity disorder and be placed in a seemingly sub-category because they havent β€˜played the game with the psychiatrist, or at least not sufficiently well. But other people with DDNOS do indeed have less obviously distinct parts of the personality and report feeling perplexed when they read about people with DID talking about their 4-year-old part called Alice or their 6-year-old boy part called Ricky. The following personal communication from someone with DDNOS (reprinted with permission) is revealing: I dont have β€˜parts like other people seem to. I have a sense of myself as being different at different times, feeling younger, or feeling aggressive or withdrawn or panicked, and its as if Im watching myself at times like this. Things come out of my mouth, stuff Im saying and I dont know why Im saying it. I can watch everything that Im saying and doing, but its like I cant do anything about it and I dont know whats going to happen next. These other β€˜parts of me arent clear though – theyre not distinct. They all respond to my name. We feel β€˜younger at these times, but I couldnt put an age on it. A lot of people dont even realise that Ive changed – I just get told that Im moody or something like that. But I know its more than that. It doesnt feel like β€˜me, and when Im like that I can remember things that I dont remember the rest of the time, although Im always worried that Im making it up. But also when Im like that, I cant do other things I normally can, like tell the time. I can just stare and stare at my watch and I know I should be able to figure it out but I just cant. Its really weird. DISSOCIATIVE AMNESIA: PART OF THE CRITERIA FOR DISSOCIATIVE IDENTITY DISORDER Then there is the whole question of amnesia. This seems to me to be a real issue that again the DSM criteria do not sufficiently address. In order to receive a diagnosis for dissociative identity disorder, you must display an Inability to recall important personal information that is too extensive to be explained by ordinary forgetfulness. It seems clear to me that this does not specify whether this inability is for current events (loss of time as it is usually experienced) or for past events (amnesia, for example for traumatic events). It is strangely vague. And yet in clinical circles, it is often taken to mean amnesia between alters, so that if the β€˜apparently normal personality (ANP) is fully co-consciousness for what other parts are saying and doing (especially the β€˜emotional personalities or EPs) then that is not β€˜full DID. And yet I know and have spoken to dozens and dozens of people with DID who are fully conscious of themselves when other parts are β€˜out. More common is amnesia for past trauma, although parts often seem to have memory for this. A very few people with apparently diagnosed dissociative identity disorder that I have met claim to have full memory of their past, with no amnesia at any time – which would seem to contradict their diagnosis. But an interesting point in this concerns the progress of therapy, which is to re-integrate traumatic memories into mainstream consciousness. At what point, when that is happening, could you state that you no longer have amnesia, and should your diagnosis change from DID to DDNOS? (And if parts are β€˜integrating or β€˜fusing during therapy, at what point should you likewise shift along the spectrum and change your diagnostic classification?) And what about instances of β€˜amnesia about amnesia – how do you know that you have amnesia for something if youve forgotten that it happened in the first place?! It all seems very muddled. DDNOS: A MATTER OF DEGREE? In terms of other differences, it seems that as a general rule the degree of the trauma or attachment difficulties leading to DDNOS will be less severe than people who are diagnosed with dissociative identity disorder, especially polyfragmented dissociative identity disorder. People with DDNOS may for example have had some β€˜good enough attachment experiences, or other mitigating factors. On a neurobiological level, differences can be seen in studies measuring the volumes of the hippocampus, a key component of the brain largely associated with memory formation and retrieval. People with DDNOS were reported to have a 13% reduction in hippocampal volume compared to healthy controls, whereas people with DID showed a reduction in the region of 25% (Ehling, Nijenhuis & Krikke, 2003). This has led clinicians such as Elizabeth Howell (2005) to suggest that the degree of dissociation correlates to the degree of severity of the trauma, which may be true. However, this is often little comfort to people with DDNOS, as I shall discuss later. All of these points present certain issues for people with the DDNOS label. Most strikingly perhaps, people labelled as DDNOS may not feel that their condition is taken as seriously as dissociative identity disorder. Some people with DID may resent the β€˜multiple personalities connotation, but at times it is the easiest way of explaining it to other people when time is short or openness limited. But how do you describe DDNOS? The temptation might be to describe it in terms of what it is lacking – Its sort of DID except not quite or Its like PTSD but with more dissociation. I wonder how many people with DDNOS therefore feel short-changed, as if somehow they are not deemed worthy of a β€˜proper condition, only a β€˜residual one, which is terribly unfair. The trauma and disorganised attachment that leads to DDNOS is incredibly severe, and so people in this area of the spectrum of dissociative distress need just as much understanding and recognition as people with dissociative identity disorder. DDNOS: NO PLACE TO BELONG? And that gives rise to the difficulties that many people with DDNOS have reported, of feeling that they dont belong anywhere. Some feel uncomfortable being lumped together with people with DID, as so often the conversation or the behaviour can revolve around the autonomy and distinctness of β€˜parts. Some people with DDNOS may prefer the company of people with dissociative identity disorder who keep their parts as much as possible in the background in public situations, but still the lack of distinct β€˜parts can be felt to be in some way as if they are getting it β€˜wrong. Of course they are not, and their experience is valid exactly because it is their experience. But that can be cold comfort, and it is a basic human need to feel that we β€˜fit in, that we have somewhere to belong. And as the β€˜DDNOS appellation is so often dropped in favour of β€˜DID – due not least to its incredibly cumbersome name, which hardly rolls off the tongue! – then people in this range of the spectrum can feel unheard, unvalidated and as if they are the only one suffering with the symptoms they have. That of course is a myth, as the vast majority of people presenting for help with a dissociative disorder, as we have seen, have a diagnosis of DDNOS. But the difficulty remains, especially as there is such a dearth of writing and literature from the perspective of people with DDNOS, who possibly feel that their viewpoint is not worth expressing, again because it is not proper dissociative identity disorder. THE STRUCTURAL MODEL OF DISSOCIATION Clinicians have also noted difficulties that arise in therapy for people with DDNOS, as opposed to DID. According to Van der Hart et als structural model of dissociation (The Haunted Self, 2006), dissociative identity disorder is a case of tertiary dissociation with multiple ANPs and multiple EPs, whereas DDNOS is a case of secondary dissociation with a single ANP and multiple EPs. At one level that is eclectic theory, but in practice it can mean that a person with DDNOS has fewer β€˜adult parts to help share the load. Where EPs are also less autonomous and less likely to be β€˜out, these traumatised parts of the personality can end up being neglected or ignored: if a person with DDNOS has non-distinctive traumatised parts of their personality, it can be harder to give them a voice and the time and space they need to bring their trauma to the fore, than it is for a clearly individuated EP with a name and age. Another issue, mentioned by a number of people without either distinct parts or amnesia, is that they have less β€˜distance and β€˜protection from the traumatic nature of their memories, or the raw emotion of the traumatised parts of themselves. Whilst someone with dissociative identity disorder might be working towards eventually narrowing the gap between their ANPs and EPs, for someone with DDNOS that gap may already be relatively narrow, and paradoxically for many this can lead to more states of crisis as they do not have the well-developed (albeit dissociative) inner resources of people with DID. Indeed, one of the hallmarks of DID is the extremes of coping capacity – in their ANP state, people with DID can present as high-functioning and extremely competent, only to crash for example at night when their EP states take over. The experience of someone with DDNOS may be fewer of these extremes, without the deep lows of trauma states of being, but also without the extreme competency of some of the avoidance-based adult parts of a DID system. Furthermore, where there is a high level of co-consciousness between different self-states in DDNOS, there is a lower risk of self-harming episodes where the adult β€˜host has (dangerously) no awareness at all of what has happened. But at the same time, shame and embarrassment also run deep, as people with DDNOS experience themselves in a semi-β€˜not-me state, but feel unable to do anything about it. In some respects, one way of looking at dissociative identity disorder is that it is a way for people to β€˜play out or β€˜act out their feelings and behaviours in another part of themselves whilst staying at a safe distance from it. So what would be otherwise unbearable feelings or thoughts can be tested out in this β€˜alternative mindspace, before gradually being reconnected with. The person with DDNOS with less elaborated parts may find this harder to do, and the perceived shamefulness of such actions and expressions may inhibit this exploration of dissociated aspects of the personality and the persons past experience. Undoubtedly, it is a mixed bag of negatives and positives for each person. IS THERE ANY VALUE IN THE DIAGNOSIS? All of this therefore begs the question of whether or not it is worth getting a diagnosis, and whether a differential diagnosis between DID and DDNOS has any value. Indeed, Spiegel et al (2011, p.841) point out the inherent flaws in the current diagnostic criteria for dissociative disorders and say: If the diagnostic criteria for dissociative identity disorder were changed to reflect the typical clinical presentation of DID (ie a complex dissociative presentation with no confirmed alter identities), these complex DDNOS patients would meet diagnostic criteria for DID. So one option, favoured by many people that I have been in contact with, is to merge the categories and to count the condition as β€˜DID/DDNOS and leave it at that. Certainly where private therapy is being sought and there is no need for a definitive statement on some official piece of paper or medical record, this may be the preferable option for a large number of people. But some people do justifiably feel the need for an β€˜official diagnosis – for a number of reasons, including the pursuit of treatment on the NHS (although a diagnosis of either DID or DDNOS is never a guarantee of appropriate therapy); in order to receive better care from the NHS than the pejorative catch-all β€˜personality disorder label will elicit; to justify or at least corroborate a claim for welfare benefits; to negotiate appropriate support from an employer; or to determine the pathway of treatment, amongst other reasons. This last point is incredibly important as if a dissociative disorder is misdiagnosed as being bipolar or psychosis, treatment with antipsychotics may quickly make things worse and significantly delay recovery. But there are a range of difficulties in gaining a diagnosis at all, not least the fact that very few NHS staff are trained to spot dissociative symptoms, let alone administer the β€˜gold standard, the SCID-D assessment tool. And in the UK, medical staff tend to prefer the diagnostic manual known as the ICD-10 (International Classification of Diseases, version 10) published by the World Health Organisation which is notoriously backward in addressing dissociative disorders. Indeed Spiegel et al (2011, p.826) in their incisive critique say: Importantly, the ICD-10 describes dissociative disorders as primarily acutedisorders that usually remit within a few weeks or months, and that have an onset in the immediate context of events that are highly stressful, traumatic, and/or that involve intolerable, insoluble problems. In contrast, the DSM-IV-TR conceptualises several dissociative disorders as long-term, chronic disorders, including dissociative identity disorder and some forms of depersonalisation disorder, dissociative amnesia, and DDNOS. They further state: The ICD-10 description of dissociative identity disorder is not based on an understanding of DID as a childhood-onset, complex, posttraumatic developmental disorder. Rather, this description is based on the literature that pre-dates the body of research on dissociative disorders since the publication of DSM-III. In short, the UK-favoured ICD-10 is based on research and clinical literature from before 1980 – little wonder that dissociative disorders are so poorly picked up in the UK. And even successfully obtaining a diagnosis can cause difficulties – in work situations, in applying for life insurance or even travel insurance, and in the stigma that surrounds so many mental health conditions. Each person needs to weigh up the pros and cons on an individual basis and do what is right for them. DDNOS AND DISSOCIATIVE IDENTITY DISORDER: SURVIVAL STRATEGIES So what is the solution? I wish β€˜the answer were easy to find, and equally easy to put into practice. In the harsh reality of the state of awareness of dissociative disorders in the UK today, we have to just keep moving in the right direction and not be dispirited at the challenges that still lay ahead. People with DDNOS need to understand that their experiences are valid and real and not inferior in any way to people with dissociative identity disorder. And whilst recognising the differences, we can also recognise the underlying similarities. In the words of Sue Richardson (2011): Both DDNOS and DID are the result of the spontaneous action of the brain in response to trauma. Both contain different self-states, holding shards of memory and β€˜unformulated experience (Stern, 1997). Both can be helped by similar approaches to therapy which encourage neuronal repair and result in brain growth such as increased hippocampal volume. Above all, all forms of dissociation need to be validated for their unique contribution to survival. It may be important for some people with DDNOS to distinguish their experience from that of people with dissociative identity disorder and it would be good for people in this category to come forwards and write about their experience to help people, clinicians in particular, understand the unique characteristics of life with DDNOS. For others, it may be validating to recognise that the distinguishing line between DDNOS and DID is largely arbitrary, and to subsume the diagnosis of dissociative identity disorder into their own self-definition of being a dissociative survivor. The important thing is that the labels people give themselves are helpful to them, to meet theirspecific needs. For some people, that means rejecting labels altogether. For others, that means fighting to have their own particular label recognised and acknowledged. As long as we have a pragmatic and even utilitarian view of diagnosis that leads people towards recovery and health, I think were near enough on the right tracks. ...See more Good morning/afternoon/evening everyone!!! Im just doing the best I can over here, but I also need to get cleaning today cuz I really really need to get this done. Im working on it. But its taking so much out of me because my apartment had gotten so very bad. So heres another article from The Mighty. Its called A Message to Emotional Abuse Survivors by Jailin Kaiser. I love you guys πŸ’œ Dear survivor, Before anything else, I want to tell you your experience is valid β€” even if you dont have bruises or scars and feel like you dont have proof. Even if you were only with them for a short time. Even if you feel like it was your fault and you had your chance to leave. You are not crazy or complaining for being honest about your experience. You are not the only one who feels this way. You are not alone in your confusion, frustration and pain. It may take you a long time to recognize you were abused. It may take years. That doesnt mean youre making it up. It may take several therapists to properly deal with your residual feelings. That doesnt mean youre being too dramatic. Someday, you might find yourself doing things your abuser used to do, and you may start to panic and think, Im turning into him/her. Youre not. Youre not like them. Youre a survivor and you can change those behaviors. You might be reading this and thinking you were crazy to leave, they werent abusive β€” just a little hard to love. You might miss them and feel guilty for abandoning them. But you didnt. You made the best decision for you, and Im proud of you. You do not need to apologize and try to make it right again β€” it never was right in the first place. Maybe youre reading this and you havent quite left yet. Please know, no matter what you decide to do, you are capable and you are worth so much. The important thing to know is you are not out of your mind, even if they convinced you that you wouldnt survive without them. They will not be the only people to ever love you. You may have been isolated from your other loved ones, but you can still repair relationships. And even if you really cant, you can still make new ones. There is a whole life after your abuser. You may find yourself with vestiges of the broken relationship. You may be apologizing just a bit too much, but know its not your fault. They might have turned everything back on you, but its not always your fault. The abuse wasnt your fault, either. Its OK to need therapists to work it out. Its OK if recovery takes a while. Its OK if youre still working through it. Its all OK β€” theres no right way to cope. You just do the best you can. Find other survivors, talk about things, make new connections. One word of caution: Please remember a relationships purpose is not to fix someone. A relationship is two whole individuals coming together to make something new and healthy, not two halves attempting to complete each other. Relationships of love should never be about a power struggle. You are not unlovable or unworthy of someones genuine affections. You are whole. You are priceless and irreplaceable. You are more than whats been done to you. I believe you can choose to become bitter or become better. And most of all, you deserve a future in which you are loved and happy. ...See more Good morning/afternoon/evening everyone!!! If you saw my response to Ravens checkin on Sunday, then what youre about to read may seem familiar...because Im literally copying and pasting my post from Sunday. I think its something important. Ready? Here we go... Have you ever found yourself overwhelmed by how bad a situation has gotten, but you dont even know how it got that bad to begin with? Questioning things like How did I end up in this position? or How did things get so messy? Well, I did a lot of cleaning this weekend...and I have no idea how my apartment got as bad as it did. But it was bad. Really bad. Realistically it still is bad, but its getting better. Heres the thing...stuff happens so slowly sometimes that you dont even realize it. I had gotten so used to just tossing my clothes at the end of my bed when I changed into my pajamas, that I hadnt even realized just how much bed Id covered!! I got so used to sleeping curled up because that was all the space I had, but theres practically a whole mattress that I was missing out on! It happened so gradually that I didnt even notice. I lost a tiny bit of my bed at a time, so it was nearly indistinguishable day to day. Now I have my bed back and its incredible to me just how big it really is. I think a lot of things happen like that. It starts off small and just gradually builds until you find yourself wondering how you let it get so bad. But thats not what happened. You didnt let anything get bad. It was tiny changes day to day that ultimately came together into this one big change or mess or whatever. Just think for a second about abusive situations. Abuse is an ongoing thing. When its mentioned in the news, youll always see at least one person asking why the person stayed/why they didnt leave/why they let it happen. Its really important to remember that it happens gradually. It starts off fine. Then maybe a few months in, they hurt you, but you brush it off because theyve been so nice until then. Surely it was a one time thing...right? I guess not. Cuz it happens again. And again. And again. And soon enough its happening so consistently you never get a break. But you cant even remember what it used to be like before by that point. Because it happened so slowly and gradually that you couldnt even tell.
